From 01 April 2018, the practice or service you use is run by Salford Primary Care Together.

The names of the practices have changed and so you will see this name on letters you might receive from your practice/service or on posters and leaflets:

-Salford Primary Care Together (Eccles)

Home Visits

Doctor's bag

Home visits are available for patients who are housebound or terminally ill.

Our clinicians typically see four patients in the practice in the time it takes to do a single home visit. For this reason, we ask our patients to come to the practice if at all possible.

If you think you might need a home visit please phone the surgery before 10am.

The receptionist will need the name, address and a contact telephone number for the patient and will also ask for brief clinical details.

The clinician on duty will contact you to decide the most appropriate time and place for you to be seen.

Please be aware that the decision to visit is made by a clinician and they need sufficient information to make this decision safely.
